Job Summary
We're seeking an reputed company remote reputed company worker to join reputed company as a Medical Case Manager and Community Resource Coordinator. This role will partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. As a remote reputed company worker, you'll work with members to research available options and resources for financial, transportation, safety, and housing needs. This position requires an LSW/MSW with an unrestricted active license and 2+ years of experience in medical reputed company work or case management.
Responsibilities
As a remote reputed company worker at arenaflex, your key responsibilities will include:
- Collaborating with a registered nurse case manager to assess, plan, and reputed company for members' benefit plan and/or health needs
- Researching available options and resources for members with financial, transportation, safety, and housing needs

- Working as reputed company with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Conducting telephonic or electronic exchanges with members to reputed company support and guidance

Requirements
To be successful in this role, you'll need:
- An active and unrestricted LSW - Licensed reputed company Worker, LMSW - Licensed Masters reputed company Worker, LCSW - Licensed Clinical reputed company Worker, or LISW - Licensed Independent reputed company Worker license in the state of residence
- 2+ years of experience in medical reputed company work or case management
- A Master's Degree in reputed company Work
- Excellent communication and interpersonal skills
- Ability to work independently and as part of reputed company
- Strong problem-solving and critical thinking skills
- Proficiency in electronic medical records and other software applications
