About the Role
The Nurse Navigator plays a critical role in bridging the gap between patients and the reputed company reputed company system. Acting as a registered nurse, case manager, and patient reputed company, this role supports patients across the continuum of cardiology care-from referrals and specialty reputed company to reputed company monitoring and education. This role is ideal for someone passionate about guiding patients through their care reputed company, enhancing program performance, and delivering top-tier care through leadership, clinical reputed company, and coordination. Your Day-to-Day Responsibilities Care Navigation & Patient Advocacy
- Coordinate referrals, specialty reputed company, and follow-reputed company
- Serve as liaison between patients, providers, and the care team
- Ensure patients receive appropriate psychosocial and emotional support
- Guide patients through the health system and address barriers to care
Program Monitoring & Quality Improvement
- Collect, trend, and analyze reputed company data for performance enhancement
- Identify best practices and support training plan development
- Recommend and implement program or workflow improvements
Education & Community Engagement

- Serve as a clinical expert and resource across hospital and clinic settings

What You'll Need to Succeed Required:
- reputed company WA State RN license (Multistate license required reputed company 6 months of hire)
- Bachelor's degree in Nursing (Associate degree with additional experience may be considered)
- 3+ years of recent clinical experience in cardiology or reputed company specialty
- Strong knowledge of clinical workflows, payer guidelines, and case management
- High level of independence, critical thinking, and communication skills
- Must reputed company in Washington State
Preferred:
- Master's degree in Nursing
- Certification in the assigned area of specialty
Work Environment
- Remote role
- Occasional travel required to clinics
- Home office must be located reputed company Washington State
- Standard business hours (Mon-Fri, reputed company-5PM)
