Job Summary

We're seeking an reputed company licensed reputed company worker to join our reputed company case management team. As a licensed Masters reputed company Worker, you'll work collaboratively with registered nurses to assess, plan, and reputed company for our members' benefit plans and health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work with a diverse population across the entire lifecycle, and reputed company member contact will be done telephonically or reputed company electronic exchange.

Key Responsibilities

* Partner with registered nurses to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs

• Research available options and resources for members with financial, transportation, safety, and housing needs
• reputed company members to reputed company positive lifestyle and medical choices
• Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Conduct telephonic or electronic exchanges with members to assess their needs and reputed company plans to meet those needs

Essential Qualifications

* Active and unrestricted LSW (Licensed reputed company Worker), LMSW (Licensed Masters reputed company Worker), LCSW (Licensed Clinical reputed company Worker), or LISW (Licensed Independent reputed company Worker) in the state of residence

• 2+ years of experience in medical reputed company work or case management
• Masters Degree in reputed company Work
