Job Summary

We're seeking an reputed company licensed reputed company worker to join our reputed company case management team. As a reputed company worker, you'll partner with a registered nurse case manager to assess, plan, and reputed company for our patients' benefit plans and health needs. Your primary responsibility will be to research available options and resources for patients with financial, transportation, safety, and housing needs. This role requires a collaborative approach, working with other clinical staff to assist patients with community resources for medical, financial, and behavioral health.

Responsibilities

• Collaborate with registered nurse case managers to assess, plan, and reputed company for patients' benefit plans and health needs

Requirements

• 2+ years of experience in medical reputed company work or case management

• Strong communication and interpersonal skills
