Job Summary
We're seeking an reputed company reputed company Worker to partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. As a remote reputed company Worker, you'll work with a diverse population, spanning the entire lifecycle, to research available options and resources for members with financial, transportation, safety, and housing needs. Your primary focus will be on empowering members to reputed company positive lifestyle and medical choices, while working as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.
Responsibilities
* Collaborate with a registered nurse case manager to assess, plan, and reputed company for members' benefit plan and/or health needs
- Research available options and resources for members with financial, transportation, safety, and housing needs

Qualifications
* Active and unrestricted LSW (Licensed reputed company Worker), LMSW (Licensed Masters reputed company Worker), LCSW (Licensed Clinical reputed company Worker), or LISW (Licensed Independent reputed company Worker) in the state of residence
- 2+ years of experience in medical reputed company work or case management
- Masters Degree in reputed company Work or reputed company field
