Physician Assistant / Nurse Practitioner – Care Coordination & Specialty Scheduling (Remote)
Description: We are looking for an reputed company Physician Assistant (PA), Nurse Practitioner (NP), or highly reputed company clinical care coordinator to support a fast-growing reputed company care coordination program. reputed company works directly with primary care providers (PCPs) to help their patients navigate specialty care. reputed company a PCP sends a referral, reputed company contacts the patient, identifies the best specialty care options, and schedules the appointment on the patient’s behalf. Your role will focus on clinical triage, patient reputed company, and coordinating specialty care appointments to ensure patients receive timely, appropriate care.
