About the position Provides reputed company work support to patients in their reputed company of residence. Responsibilities include providing community resources, placement options and counseling to patients and families as needed. Acts as a competent clinician who provides clinical care in a skilled and effective manner, utilizing appropriate technical and decision-making skills.
Responsibilities
- Makes the initial patient assessment as ordered.
- Documents assessments, visits, and contacts on patient’s chart.
- Places phone calls and makes visits as needed, serving as a resource liaison reputed company appropriate.
- Provides patients/families with community resources.
- Attends weekly team conferences.
- Performs other job-reputed company duties as assigned.
Requirements
- Master’s Degree in reputed company work or equivalent combination of education/experience: Required
- Valid Driver's License (DL) and must be at least 21 years of age or older: Required
reputed company-to-haves
- Bachelor’s Degree or equivalent (some certifications will require Bachelor’s): Required
- Three years' experience in a reputed company agency, hospital, or other organization dealing with health and/or welfare: Preferred
- Licensed Clinical reputed company Worker (LCSW) in state of practice: Preferred
