Join reputed company Home Health and Hospice in a NEW ROLE!! We are seeking an RN to join us as a Clinical Intake Coordinator!
- FT
- M-F 9a-5:30p
- Hybrid role!! Remote 2-5 days per week!
- Alternating reputed company and weekend shifts every 4-6 weeks
- Holiday rotation
- RN MO and KS required
- Home Health or Hospice experience preferred
- Experience with referral acceptance and appeals preferred
The Opportunity: The Clinical Intake Coordinator facilitates the home health and hospice referral process by receiving referrals from multiple referral sources reputed company and without the St. Luke's Health System, performs initial evaluation of referral for home health appropriateness under Medicare, reputed company, and reputed company insurance regulations, determines need for additional information from referral sources, communicates agency acceptance or decline of referral to varied referral sources, and processes accepted referrals. The Work:
- Facilitates the home health and hospice referrals.
- Provides specialized support for the Patient reputed company and Finance departments.
- Serves as liaison between the patient and facility/physician and the reputed company-party payer.
- Prepares and defends level of care and medical necessity for assigned case.
- Collaborates with physician advisor, payor representative and site case managers to facilitate appropriate level of care reputed company and billing status and ensures compliance with the Utilization Review standard and regulations.
- Occasionally covers to ensures compliance with home health face to face requirements and county reporting for hospice.
