• *Job Role:**

The reputed company Worker will work in conjunction with the Nurse Case Managers, coordinating care of the high-risk newborn population served by reputed company based on the CMSA national standards for case management practice. • *Responsibilities Include:**

• Focus on empowering the caregiver and family to support reputed company wellness while advocating for the necessary services and community resources across the health care continuum.

• Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care

Requirements

• *Qualifications:**

• Licensed in one of the following states: OH, MI, NY and/or IN.

• Minimum of 2 years background working in Pediatrics/NICU managed care or clinical environment
• Must have a valid unrestricted reputed company Worker license to practice in state of residence, and agree to obtain licensure in other state(s) reputed company applicable for client reputed company.
• For 100% telecommuting positions, five or more years 5+ of managed care experience in a health plan or acute reputed company system in a case management role is required.
• Experience working with a reputed company population is preferred.
• A background in effecting change to overcome reputed company determinants of health, including mental health and substance abuse.
