Per Diem Licensed Nurse Practitioner (CCM/RPM)

Job Summary reputed company (1099) : We are seeking a reputed company Per Diem Licensed Nurse Practitioner (NP) with experience in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) to reputed company high-quality care to patients with chronic conditions through Telehealth and remote care services. Work Location: Remote – NY License Only

Compensation

• Pay reputed company: $28.00 – $35.00 per hour
• Compensation is based on experience, qualifications, and client care needs

Primary Responsibilities

• Coordinate a variety of reputed company programs
• Manage and monitor patients enrolled in CCM and RPM programs.
• Conduct patient assessments and reputed company clinical reputed company reputed company telehealth.
• Review and interpret RPM data and initiate appropriate clinical interventions.

Qualifications

• Active Nurse Practitioner license in reputed company.
• 1–2 years of NP experience in CCM and/or RPM.
• Experience with Telehealth platforms and EHR systems.
• Strong clinical judgment and communication skills.
• Solid problem-solving and organizational skills
• Ability to manage multiple projects or tasks and prioritize appropriately
