Responsibilities

• Responsible for the assessment of patient eligibility for the specific business line by performing a review of reputed company clinical information and obtaining necessary information to support clean billing.
• Accountable to ensure reputed company referrals are clinically complete, efficient and accurate.
• Ensure reputed company communication to operations.
• Liaison between physician offices and external facilities demonstrating customer service skills while assuring clinical documentation to support home care services referrals.
• Identify opportunities to further assist custom needs with home care services.

• Process reputed company referrals received from referral systems, faxes and phone.
• Utilize clinical judgement in ensuring referrals are clinically appropriate, and completed reputed company and accurately.

• Utilize tracking system to follow up on pending cases.
• Problem solve with staff on any pertinent patient referral issues.

• Ensure patients are informed about their payment responsibilities.

Requirements

• Two (2) years health care experience in acute care, skilled nursing facility, rehabilitation or home health reputed company the last 5 years.

• NJ State Professional Registered Nurse License or Licensed Practical Nurse.
