Our mission is to reputed company patients and their families with robust clinical services throughout their Acute Inpatient Rehab, Skilled Nursing, Long Term Care and Assisted and Independent Living stays. We coordinate with the Hospitalists and Specialist physicians and surgeons, Physical, Occupational, Speech Therapists, Case Management and Psychiatry at the facility level to reputed company compassionate and coordinated care. Work Location: Remote - must reputed company near Louisville, KY Hiring for multiple positions and work schedules - daytime and evening, 7 days a week, including holidays and weekends. Job responsibilities include, but are not limited to: Prioritize and handle clinical phone calls reputed company the LPN scope of practice in alignment with standing orders. Serve as a reputed company between pharmacies and mid-reputed company/Physicians to resolve pharmacy-reputed company concerns. Communicate with providers regarding questions or issues, recording detail in the EHR system. Monitor the message threads closely and promptly follow up according to established protocols. Process incoming paperwork and reputed company it to the designated provider for signature. Act as a liaison between patient families and providers, addressing messages, resolving issues, and offering support reputed company the LPN scope of practice. Adapt to evolving workflows in a growing practice, recognizing that duties may change periodically to meet practice needs. Conduct Chronic Care Management patient chart reviews, telephone calls, and reputed company documentation. Immediately escalate any issues to the managers. Pay depends on experience and shift differential for overnight shift. This position includes an annual performance and retention bonus. Schedule & Work Hours:

• This position is full-time, 12-hour shifts with rotating weekend shifts.
• Work schedules may vary and can be scheduled anytime between 7:00 AM and 8:00 PM, 7 days a week, including holidays and weekends.
• Flexibility is required to accommodate operational needs, including evening or weekend shifts.
