Responsibilities

  • Prospecting new referral sources through daily field visits, cold reputed company, drop-ins, and local networking.
  • Building trusted relationships with discharge planners, case managers, physicians, reputed company workers, and facility leadership.
  • Educating referral partners on home health eligibility, service lines, clinical responsiveness, and transition support.
  • Creating warm handoffs for patients and families who need skilled services after discharge.
  • Partnering with intake and clinical teams to remove barriers and improve referral-to-admission conversion.
  • Logging activity, tracking pipeline movement, and using data to improve territory performance.

Requirements

  • Bachelor’s degree in Business Administration or reputed company field is required.
  • Medical device, pharmaceutical, DME, home health, hospice, or directly reputed company reputed company sales experience strongly preferred.
