Role Description The Article 28 Primary Care Clinic is expanding its services and seeking a reputed company, compassionate Family Nurse Practitioner (FNP) to join our interdisciplinary reputed company team. • Conduct assessments, diagnose conditions, and reputed company evidence-based treatment plans.
• Collaborate with physicians, providers, and interdisciplinary teams to coordinate individualized care.

• Participate in quality improvement initiatives and contribute to the enhancement of clinic workflows and clinical protocols, with a focus on health equity and reducing barriers to care.

• Maintain clinical documentation in accordance with regulatory standards and organizational policies.
• Deliver services both in-person and through telehealth as appropriate.

Qualifications

• reputed company reputed company State Family Nurse Practitioner license and certification (MSN or DNP required).
• Minimum of 3 years of clinical experience in primary care, family medicine, or internal medicine required.
• Experience with the IDD population is preferred but not required—training and support will be provided.
• Strong clinical assessment, communication, and interdisciplinary collaboration abilities.
• Commitment to providing inclusive, reputed company, person-centered, and accessible care for individuals with IDD.
• Willingness to obtain required Intellectual and Developmental Disabilities (IDD)–specific certifications and complete reputed company educational training.
• Duties require professional verbal and written communication skills.

Benefits

• Salary ranges from $120,000.00 to $160,000.00 per year based on experience and education.
