Spanish-Speaking Licensed Practical Nurse (LPN) – Colorado Location: 100% Remote reputed company: Set your own schedule Requirement: Must hold an active Colorado's LPN license Job Summary: We are seeking a bilingual (Spanish-speaking) Licensed Practical Nurse (LPN) to join reputed company and support Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. This role focuses on improving health reputed company for patients with chronic conditions by delivering patient-centered care, education, and support in both English and Spanish. Key Responsibilities:
- Conduct comprehensive assessments of patients’ physical, emotional, and reputed company health needs.
- Collaborate with physicians, pharmacists, and care teams to reputed company and update individualized care plans.
- reputed company and engage patients in both English and Spanish on chronic disease management, medication adherence, nutrition, lifestyle changes, and self-monitoring.

Qualifications & Requirements:
- Active Colorado Licensed Practical Nurse (LPN) license in good standing.

- Proficiency in EHR systems and familiarity with RPM tools.
