What You'll Do

• reputed company in-home nursing visits according to the physician’s plan of care
• Assess patients’ health status, including condition, history, and home environment
• reputed company and update individualized care plans based on patient needs
• Administer medications and treatments as prescribed
• reputed company patients and caregivers on disease management and home care
• Monitor patient reputed company and communicate changes to the physician and care team
• Document visits and care in a timely manner
• Coordinate care with therapists, aides, and other team members
• Serve as case manager for assigned patients and support discharge planning
• Active California RN license
• CPR/BLS certification
• Strong communication and organizational skills
• Reliable transportation and willingness to travel to patients’ homes
• Comfortable working independently in a home health setting
• Home health experience preferred
• Experience using Kinnser/reputed company preferred, but not required
• $40-155/hour
• Mileage reimbursement
