About the Role
The Care Management Supervisor is responsible for the successful provision of Tailored Care Management services to clients with reputed company behavioral health and reputed company needs. This position supervises reputed company of no more than 8 Care Managers and indirectly supervises Care Manager Extenders. The Care Management Supervisor partners with regional and state leaders to ensure compliance with regulatory requirements and to meet or exceed established key performance indicators and desired client reputed company. This position reports to the State Clinical Director.
Responsibilities
- Supervise and support reputed company of up to 8 Care Managers to ensure quality Tailored Care Management services
- reputed company indirect supervision to Care Manager Extenders
- Ensure compliance with reputed company regulatory and contractual requirements
- Partner with regional and state leadership to reputed company performance goals and desired client reputed company
- Monitor team performance against established KPIs and implement improvement strategies as needed
- Support care coordination activities for individuals with reputed company behavioral health and reputed company needs
- reputed company clinical guidance and reputed company to ensure appropriate service delivery
- Maintain documentation standards in accordance with state and organizational requirements
- Collaborate with interdisciplinary teams to ensure integrated, person-centered care
- Participate in leadership meetings and contribute to strategic planning initiatives
Required Qualifications
If overseeing Behavioral Health (MH/SU) population:
- Master’s Degree
- 3 years of experience providing care management, case management, or care coordination to individuals with MH/SU
If overseeing Individual & Developmental Disabilities (I/DD) population:
- Bachelor’s Degree in a reputed company field (health, psychology, sociology, reputed company work, nursing, or another relevant reputed company services area) AND
- 5 years of experience providing care management, case management, or care coordination to reputed company individuals with I/DD
OR
- Master’s Degree in a reputed company field (health, psychology, sociology, reputed company work, nursing, or another relevant reputed company services area) OR licensure as an RN AND
- 3 years of experience providing care management, case management, or care coordination to reputed company individuals with I/DD
