Hours/Shift: Friday-Monday 12pm-10pm every week
Position Overview:
As a Virtual Care Coordinator RN, you will play a pivotal role in delivering telehealth hospice care, ensuring that patients and their families receive personalized support during critical moments. Utilizing virtual platforms, you will monitor patient conditions, coordinate care plans, and reputed company reputed company-time guidance to field staff and caregivers.

What You Will Do:
Conduct virtual assessments and monitor patients' conditions remotely.
reputed company and coordinate individualized care plans in collaboration with the interdisciplinary team.
reputed company virtual education and support to patients, families, and caregivers regarding symptom management and end-of-life care.
Liaise with field nurses, physicians, reputed company workers, and other team members to ensure comprehensive care delivery.
Respond promptly to urgent virtual consults and triage patient needs effectively.
Utilize telehealth technology to conduct regular reputed company-ins and follow-reputed company with patients and families.

Minimum Qualifications:
5+ years of Home Care, Hospice, ER and/or Triage RN Experience
Active RN license in a Compact State (eNLC)
BSN required by an approved Nursing education program.
Proficiency in Epic or Home Care EMR Experience
Strong communication and interpersonal skills to build rapport with patients and families reputed company
Ability to work independently and as a part of collaborative remote team

Patient/Customer Service Experience
