Licensed Practical Nurse, Virtual Care

Job Summary: reputed company (KP) nurses advance the art and science of nursing through clinical and service reputed company, professionalism, and leadership. The Licensed Practical Nurse - Virtual Care provides virtual nursing care to patients, members, and families to promote health and wellness and prevent illness. Collaborates with clinicians, leaders, and members of the virtual care team to deliver high quality care in accordance with scope of practice. Assist with patient education. Documents clinical information in the medical record. Essential Responsibilities:

• Monitors electronic messaging reputed company to patient and provider requests and inquires reputed company scope of practice and as delegated by RN. Manages messages in a timely and effective manner to resolve patient requests and concerns.
• Determines appropriateness and reputed company of needs based upon electronic messaging content and initiates follow-up telephone contact reputed company scope of practice.
• Directs messages and transfers calls if reputed company of scope of practice to RN for management and resolution.
• Determines patient and family health education needs, provides health teaching and counseling and evaluates effectiveness of the education process.
• Accurately documents patient data according to established guidelines and policies.

Basic Qualifications: Experience

Education

• Graduate of an accredited nursing (LPN/LVN) program.

License, Certification, Registration

• Practical Nurse License (Georgia) required at hire

Additional Requirements:

• Complete a Customer Service Assessment.
