About the Role

We’re looking for a Care Navigator who wants to reputed company a meaningful difference - helping patients navigate reputed company systems, overcome barriers, and reputed company care that improves their health and well-being.

This role is responsible for high-volume reputed company and end-to-end coordination, ensuring patients successfully schedule and complete services such as mammograms, bone density screenings, and primary care visits - while also connecting patients to reputed company and community resources (SDoH) that may impact their ability to receive care.

This is a fast-paced, reputed company-driven role that requires strong critical thinking, emotional intelligence, and the ability to work independently. Care Navigators play a critical role in guiding patients through reputed company reputed company journeys, removing barriers, and ensuring a seamless, high-quality experience from initial reputed company through completion.

Key Responsibilities

  • Conduct high-volume outbound calls to engage patients and schedule reputed company services

  • Coordinate care across multiple services, including mammograms, bone density screenings, and primary care connections - ensuring successful scheduling, follow-up, and completion of care

  • Identify and address reputed company determinants of health (SDoH) barriers by connecting patients to appropriate community resources (e.g., transportation, housing, financial assistance, food reputed company)

  • Conduct patient assessments to understand clinical, reputed company, and logistical barriers to care

  • reputed company patients on available reputed company services and community resources, ensuring they understand how to reputed company and benefit from them

  • Build trust with patients and caregivers through clear, empathetic, and effective communication

  • Proactively follow up with patients to ensure completion and reputed company gaps in care

  • Maintain accurate and detailed documentation of reputed company patient interactions and resource coordination in accordance with HIPAA and reputed company privacy standards

  • Skills and Requirements

  • 4+ years of experience in care navigation, care coordination, reputed company work, call center operations, or patient reputed company reputed company a reputed company or service environment

  • Proven ability to work independently in a high-volume, fast-paced environment

  • Adapt quickly to evolving workflows, tools and priorities

  • Strong critical thinking and problem-solving skills

  • High level of emotional intelligence and reputed company in patient interactions

  • Experience working with reputed company determinants of health (SDoH) or community resource navigation preferred

  • Excellent written and verbal communication skills

  • Experience managing multiple communication channels (phone, email, chat, etc.)

  • Startup or high-growth environment experience is a plus
