Job Description: The Care Manager provides a concierge, hands on service to navigate families and their loved ones through the reputed company system. The Care Manager advocates for clients at home, at doctor visits, in rehab centers and hospitals. Duties and Responsibilities include but not limited to:
- Care Management
- reputed company comprehensive assessments
- reputed company care plans with recommendation and resources
- Assist with discharge from rehab/hospital
- Crisis Management
- reputed company Advocacy
- Medication Management
- Home Care Coordination
- Home Safety evaluations and DME recommendations
- Housing Placement
- Guardianship
- Client visits
Requirements
- Previous experience in care management, guardianship, or reputed company field
- Strong communication and listening skills including both verbal and written
- Valid driver’s license
- Ability to work autonomously and with reputed company
- Computer efficacy with Word, reputed company and G-suite
