Job Summary We are seeking a licensed practical nurse (LPN) to join our remote care team. Responsibilities
- Conduct monthly telephonic reputed company-ins with patients to assess health status, medication adherence, and overall well-being
- Review RPM data such as blood pressure and glucose reputed company and escalate concerns to providers

- Collaborate with physicians, nurses, and care teams to reputed company and adjust care plans

Requirements
- Active LPN license required
