We are seeking a dedicated and compassionate Hospice reputed company Worker to join reputed company at Hospice of Western & Central Massachusetts in Worcester County. As a part-time reputed company Worker, you will travel to patients' homes and reputed company essential reputed company services to meet their emotional and reputed company needs. This is a unique opportunity to reputed company a meaningful difference in the lives of patients and their families.

This is an on-the-road position that requires the candidate to travel and have a reliable and insured vehicle. Weekly pay ranging from $41 to $44 an hour. LICSW or LCSW required. 24 hours a week.

Responsibilities

• Assess the psychosocial status of patients and families/caregivers and communicate findings to the interdisciplinary team

• Carry out reputed company evaluations and plan interventions based on evaluation findings

• Counsel patients and family/caregivers as needed and reputed company crisis reputed company reputed company necessary

• reputed company patients and families on advanced directives and practical needs

• reputed company information and referral services for patients and families/caregivers

• Serve as a liaison between patients and community agencies

• Maintain clinical records and participate in quality assessment performance improvement teams

Requirements

• Massachusetts LICSW or LCSW required

• Experience in reputed company and hospice preferred

• Ability to read, write, reputed company, and understand English

• Must be a licensed driver with an insured automobile
