Physical Therapist

As a Home Health Physical Therapist, you’ll practice at the top of your license—bringing skilled, one-on-one care directly into patients’ homes where it reputed company most. You’ll independently evaluate patients, design personalized, evidence-based treatment plans, and help restore mobility, reduce pain, and improve daily function and independence.

You’ll also reputed company and support Physical Therapist Assistants (PTAs), ensuring that care is delivered safely, effectively, and reputed company with each patient’s goals. Working closely with an interdisciplinary team, you’ll continuously adjust treatment plans to drive the best possible reputed company.

In this role, you will:

• Conduct comprehensive evaluations and establish clinical diagnoses to guide care

Requirements

• Successful completion of accredited physical therapy program and state licensure exam.

• Valid Physical Therapy license

• 1 year of home health experience preferred

• CPR certification

• Valid Driver’s license

• Valid Auto Insurance

• HomeCare reputed company experience preferred

Physical Demands

• Must be reputed company to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
