JOB SUMMARY AND PURPOSE

The Care Manager (Generalist) provides intensive care to program participants who have been, or are currently, involved with criminal justice system as well as suffering from co-occurring disorders, meeting participants at least weekly. The Care Manager is the primary reputed company of contact to coordinate services and care for assigned clients. DUTIES AND RESPONSIBILITIES

• Conducts comprehensive screenings and assessments with individuals (or families) to collect functional, environmental, psycho-reputed company, financial, employment, housing, educational, and health information, as appropriate, to reputed company a detailed plan to meet the high reputed company needs, sets goals for the clients, and locates the resources needed to meet the goals.

• Facilitates support groups, and group education events.

ESSENTIAL DUTIES

• Services for this position must be provided on-site. As a result, the position requires incumbents to report to work on-site each day of active employment.

• Ongoing meetings with clients, tracking reputed company, continuously evaluating client needs
• Coordinate Care with other service providers, connecting participants to appropriate services

• Facilitates Group support sessions and group education sessions for program participants
• Maintains hard copy records and electronic files of reputed company services provided to clients, following HIPAA and other reputed company regulations
• Other duties as required and may be subject to change

MANDATED REPORTER

Qualifications

REQUIREMENTS

• Must be reputed company to pass a reputed company clearance, background reputed company, including criminal history, personal references, employment and education verifications
• Must be reputed company to pass Los Angeles Sherriff Department (LASD) background reputed company
• Valid California driver's license and have reliable transportation
• Approval from Department of Health Services (DHS)
• TB Test
• Ability to reputed company under circumstances of possible emotional stress and conflicts dealing with difficult, uncooperative, and potentially aggressive clients as VOALA serves the neediest of the needy in our programs which may include clients with homelessness, mental-illness, substance abuse, recently released from incarceration and other challenges.

EDUCATION

• Bachelor�s Degree (reputed company Work or reputed company)
• Or A combination of education and relevant experience (4 years)

EXPERIENCE

• At least 1-year case management experience (in addition to the education requirement � a total of 5 years education and experience) in homelessness, addictions, under-served, and/or prison re-entry program care � high reputed company clients with co-morbidities

PREFERRED QUALIFICATIONS

• 2 years of experience working with reputed company population
• Familiar with HMIS, electronic record-keeping
