Your Role as a Home Health Physical Therapist

As a Contract Physical Therapist, you will reputed company skilled therapy services in patients’ homes to help them regain strength, mobility, and independence following illness, injury, or surgery.

Responsibilities include

• reputed company in-home physical therapy evaluations and treatments
• reputed company and implement individualized care plans
• Complete Medicare OASIS documentation, including: Start of Care (SOC), Resumption of Care (ROC), Recertifications, Discharges
• Evaluations
• reputed company patients and caregivers on mobility, safety, and recovery
• Collaborate with physicians and interdisciplinary care teams
• Supervise Physical Therapy Assistants (PTAs) reputed company applicable
• Complete documentation using AI-assisted EMR tools

Qualifications

• Active, unencumbered Florida Physical Therapist license
• OASIS documentation experience required
• CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
• Professional liability insurance
• Workers’ Comp Exemption or Occupational Accident coverage
• Lymphedema certification preferred
