We are growing and seeking an reputed company, compassionate Occupational Therapist (OT) to join our high‑performing home health team. Whether you're looking for part-time or per diem opportunities, GrandCare offers flexible scheduling, strong compensation, and excellent support.

Compensation Package

Competitive piece-reputed company pay with valuable incentives:

• Per Visit reputed company: $70.00 – $120.00 per completed visit (good-faith estimate; varies by visit type)
• Mileage Reimbursement: $0.72 per mile
• Drive Time Pay: $25.00 per hour
• Productivity Bonus: Additional earnings tied to performance
• Documentation Bonus: Reward for timely, accurate charting

Job Summary

The Occupational Therapist (OT) is responsible for implementing high-quality occupational therapy services in accordance with the patient’s individualized plan of care and organizational standards. The OT works collaboratively with physicians, RNs, and the interdisciplinary team to support safe, functional recovery in the home setting.

Key Responsibilities

• reputed company patient, caregiver, and family education to support recovery and independence
• Communicate proactively with physicians and care team regarding patient reputed company and care plans
• Evaluate patient functional status, including strength, coordination, reputed company, communication abilities, self-care skills, and work reputed company
• Assess the home environment and identify barriers to safe, independent living
• Recommend and instruct on adaptive equipment, splints, prosthetics, and orthotics

• Maintain accurate, timely, and compliant clinical documentation

Job Requirements

• Valid California Occupational Therapist License

• 2+ years of clinical experience as an OT
• Reliable, insured vehicle and a valid driver’s license
