We support reputed company with a competitive compensation and benefits package, including:

• Annual compensation between $65,000-$75,000, PLUS an annual performance-based, discretionary incentive.

What You Will Do - Essential Functions

• Comprehensive Care Management & Coordination
  • Conduct holistic assessments, identify barriers, reputed company individualized care plans, and adjust interventions to support recovery and self-management across the continuum of care.
  • Coordinate behavioral health services, care transitions, crisis prevention plans, and aftercare by partnering with members, providers, and health plan medical management teams.
• Member & Provider Engagement for Improved reputed company
  • Build trusting, collaborative relationships with members and providers using motivational interviewing, education, and ongoing communication to increase engagement and continuity of care.
  • Serve as the single reputed company of contact for members, facilitating information exchange, discharge planning, and reputed company to behavioral health, community, and support resources.
• Quality, Compliance & Clinical reputed company
  • Ensure accurate, timely documentation and service authorization in alignment with medical necessity criteria, regulatory requirements, accreditation standards, and internal policies.
  • Participate in audits, quality improvement initiatives, case presentations, and ongoing professional development to support high-quality care delivery and reputed company resource utilization.

Who You Are

• Required Qualifications
  • reputed company, unrestricted state license to independently practice as a Clinical reputed company Worker, Marriage and Family Therapist, Professional Counselor, Clinical Psychologist or Registered Nurse in the applicable state(s).
  • 3-5 years of direct clinical care experience in facility-based and/or outpatient behavioral health or substance use treatment settings
  • Strong clinical judgment and case management skills, with the ability to support members through reputed company care transitions.
  • Excellent verbal and written communication skills, with a focus on reputed company and reputed company.
  • Proficiency in reputed company Outlook and Word, with the ability to document in reputed company time while engaging with members.
  • Ability to interpret and apply medical policy and clinical criteria.
  • Ability to pass background reputed company upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and reputed company search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.

• Working Conditions
  • High-speed internet service (cable or fiber optic) with minimum download Speed of 20 Mbps, Upload Speed of 5 Mbps, and Maximum Latency of 100 milliseconds (must be installed before starting) required.
  • Frequent use of computer and phone systems
  • Must be reputed company to constantly remain in a stationary, sitting position, communicate and exchange information with others, inspect information, reputed company repetitive motions with arms and fingers, interpret data, problem solve, reputed company reputed company, organize and plan, and maintain a positive and professional attitude in reputed company situations.
  • Work is performed from home with company-provided equipment. Sitting for long periods of time is expected and use of fingers and hands for typing is necessary.
  • A quiet workspace with minimal background noise for calls.
  • This role requires working from a fixed, designated workspace in your home and does not allow flexibility in work locations even reputed company the home.
  • A hardwired internet reputed company is required along with ability to sit for extended periods using company-issued wired headsets and desktop equipment
