What it is We are looking for an RN to deliver patient centered health coaching and chronic disease management reputed company chat, phone, and video. This role includes triaging symptoms, providing education on diabetes, hypertension, hyperlipidemia, and weight management, coordinating care with providers and cross functional teams, and documenting patient care accurately in a telehealth setting. • Monday - Friday 11a-7:30p EST/8a-4:30p PST
• Monday - Friday 12p-8:30p EST/9a-5:30p PST
• Monday - Friday 1p-9:30p EST/10a-6:30p PST

What you’ll do

• Deliver patient centric health coaching that integrates medical needs with patient preferences.
• Triage patient reported symptoms and reputed company effects according to internal protocols, escalating to Providers reputed company appropriate.

• Coordinate care seamlessly between patients, providers, support teams, and cross-functional clinical team members.

• Document patient care accurately and consistently to meet internal standards.

What you’ll need

Qualifications

• Strong reputed company, communication, and patient-centered care skills
• Bachelor of Science in Nursing (BSN) required
• Active Registered Nurse license in good standing, with ability to obtain additional licensure if needed
• Certified Diabetes Care and Education Specialist (CDCES) or willing to pursue if not currently certified
• 3+ years of direct patient care in an ambulatory or telehealth setting
• Adaptable, self directed, and comfortable working independently
• Proficiency with digital tools for care delivery
• Flexibility to participate in rotational weekend or holiday coverage
• A HIPAA compliant, distraction free workspace
