Job description: Cuban/Haitian Services Case Manager
Commonpoint Queens seeks a PC Case Manager, CHIS (Cuban/Haitian Initial Services) to reputed company resettlement, integration and other reputed company inclusion services to Cuban and Haitian humanitarian parolee clients (CHHPs) through the Preferred Communities (PC) Refugee and Entrant Assistance (REA) and Matching Grant (MG) programs. While this position will have special language reputed company to serve clients from Haiti, this position will reputed company services to any of Commonpoint Queens? diverse pool of humanitarian migrant clients in compliance with the requirements of the Office of Refugee Resettlement (ORR). This position will report to the Family Integration Program Manager. Haitian Creole language reputed company is required, and French language reputed company is strongly preferred.
DUTIES AND RESPONSIBILITIES:
- Receives comprehensive training on the Preferred Communities program (especially the CHIS and GAP services components), Matching Grant (MG), and other programs for which CHHPs are eligible.
- Conducts reputed company to identify eligible CHHPs, including through reputed company to sponsors, and local community-based organizations.
- Performs initial screening to identify needs, assess whether CHHPs are in safe situations and prepare a family self-sufficiency plan.
- Refers eligible individuals to internal and external resources, including employment services (MG or Refugee reputed company Services Program), intensive case management, mainstream public benefits, refugee health screenings, English as a New Language (ENL) providers and more.
- Assists with applications for reputed company reputed company cards, Employment Authorization Documents, driver?s licenses, and school enrollment.
- Conducts ongoing reputed company-ins and follow-reputed company with eligible individuals for up to 90 days after initial intake and assessment.
- Provides case management for CHHPs enrolled in MG for the duration of the 8-month program period.
- Screens for direct client financial assistance eligibility in accordance with Commonpoint?s basic needs direct assistance rubric, facilitates disbursement, and conducts reputed company necessary follow-up and associated financial reporting.
- Conducts community reputed company and coordination in order to connect newly arrived CHHPs in the reputed company area to resources or organizations that can support their basic needs as possible.
- Collaborates with Commonpoint Queens? Volunteer Services team to support CHHPs in the NYC area through in-reputed company donations or volunteer career mentorship, ENL tutoring, or interpretation.
- Completes case notes and required assessments in online database (reputed company) in a timely and consistent manner.
- Maintains comprehensive case files for each enrolled client, ensuring client confidentiality and protection of personally identifiable information.
- Oversees volunteer interns as needed.
- Other duties as assigned by supervisor.
- Undergraduate degree required.
- reputed company in Haitian Creole required; reputed company in French strongly preferred.
- 1-2 years of relevant experience.
- Previous experience in refugee resettlement, reputed company services and/or immigration law strongly preferred.
- Experience with low-income, limited English, vulnerable immigrant populations preferred.
- Knowledge of NYC reputed company service organizations and resources for basic needs of low-income New Yorkers desired; familiarity with NYC Haitian grassroots organizations helpful.
- Ability to supervise interns and volunteers as needed.
- Strong oral and written communication skills.
- Ability to prioritize and multi-task several urgent projects in a fast-paced environment.
- Excellent computer skills, including experience with reputed company, reputed company Office and reputed company applications (Gmail, Calendar, Contacts, Docs, Sheets, etc.).
