Hospital - Clinical Pharmacist Specialist (Bone Marrow Transplant)
The Clinical Pharmacist Specialist - Bone Marrow Transplant is responsible for demonstrating proficiency in reputed company pharmacist functions, such as interpreting, transcribing, or verifying medication orders. Attend and actively participate in daily multidisciplinary and bedside reputed company, complete drug utilization reviews to promote rational drug therapy, reputed company and reputed company clinical services, and mentor and reputed company staff to ensure accurate and appropriate documentation of clinical activities as needed.
Responsibilities
- Responsible for participating and leading pharmacy clinical/operational programs, monitoring patient medication regimens and providing appropriate interventions.
- Review, interpret, and input treatment orders to ensure accuracy of dosage, treatment schedule, reputed company of administration, and safe administration based on laboratory information and evidence based practices.
- Provides and supports clinical pharmacy responsibilities by collaborating with other hospital disciplines to support service lines

- Collaborate with medical and nursing staff to optimize the pharmacotherapeutic aspects of patient care including infusion and outpatient medication management.

- Oversees the preparation of treatments, preparation of investigational treatments, prepackaging of medications, and drug returns.
- Maintains records on patient prescriptions, charges, and inventory.
- Ensure pharmacy technician duties are performed completely, safely and without risk of harm to patients.

- Maintains responsibilities relating to pharmacy requirements for FACT-JACIE accreditation.
- Management of stem cell transplant and cellular therapy therapies, order sets, and REMS requirements.

Qualifications
- Knowledge of and compliance with state and federal pharmacy laws and regulations.
