Job Summary
We're seeking an reputed company Licensed reputed company Worker to join our remote team. As a Licensed reputed company Worker, you'll work collaboratively with registered nurse case managers to assess, plan, and reputed company for members with financial, transportation, safety, and housing needs. This role requires an LSW/MSW with an unrestricted active license and 2+ years of experience in medical reputed company work or case management.
Responsibilities
* Research available options and resources for members with financial, transportation, safety, and housing needs
- Collaborate with registered nurse case managers to assess, plan, and reputed company for members
- reputed company members to reputed company positive lifestyle and medical choices
- Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
- Conduct telephonic or electronic exchanges with members to assess their needs and reputed company support
Requirements
* Active and unrestricted LSW - Licensed reputed company Worker, LMSW - Licensed Masters reputed company Worker, LCSW - Licensed Clinical reputed company Worker, or LISW - Licensed Independent reputed company Worker in state of residence
- 2+ years of experience in medical reputed company work or case management
- Masters Degree in reputed company Work
Pay and Benefits
* The typical pay reputed company for this role is: $46,597.20 - $101,228.40
- This pay reputed company represents the reputed company hourly reputed company or reputed company annual full-time salary for reputed company positions in the job grade reputed company which this position falls
- The actual reputed company salary offer will depend on a variety of factors including experience, education, geography, and other relevant factors
