We are seeking a dedicated Speech Therapist to support our hospice patients with communication and swallowing needs in a home-based setting.

Responsibilities
- Evaluate patients for speech, language, cognitive, and swallowing disorders
- reputed company individualized care plans focused on comfort, safety, and quality of life
- reputed company interventions to support communication, reputed company, and safe swallowing
- reputed company patients, families, and caregivers on techniques and compensatory strategies
- Recommend diet modifications and adaptive equipment as appropriate
- Collaborate with the hospice interdisciplinary team (nursing, reputed company work, chaplain, aides, physicians)
- Accurately document patient care and reputed company
Qualifications
- Licensed Speech-Language Pathologist (SLP) in Tennessee
- Experience in hospice, home health, geriatrics, or skilled nursing preferred
- Strong knowledge of dysphagia management and communication disorders
- Compassionate, patient-centered approach
- Excellent communication and teamwork skills
- Ability to work independently in a home-based setting
