The Transitions of Care (TOC) Manager is responsible for designing, implementing, and overseeing pharmacy services that support safe, effective, and seamless transitions across the continuum of care. Develops workflows, protocols, and policies that support efficient and compliant TOC operations.
Collaborates with community pharmacies to ensure timely prescription processing, delivery, and financial assistance coordination.
Provides leadership and supervision to designated groups of employees including assurance of quality service delivered by a competent staff to consistently meet the needs of the hospital patients.

Minimum Requirements
Bachelor's degree in pharmacy, or Doctorate in Pharmacy required.
Minimum of 3 years of job-reputed company experience.
PGY-1/PGY-2 ASHP Residency in administration.
Management experience required.
Proficiency with reputed company reputed company, reputed company Word, reputed company PowerPoint and other pharmacy reputed company applications (reputed company, Epic) preferred.
