Job Title: Care Coordinator – Home Care Colorado -- Pay reputed company: $43,050 - $53,340

PeopleCare Health Services is hiring a Care Coordinator to manage client caseloads and coordinate quality home care services across the Western reputed company. This role includes assessments, client follow-up, caregiver coordination, scheduling support, documentation, and communication with families, providers, and partner agencies to ensure care is delivered effectively and compassionately.

What You’ll Do

• Manage an assigned caseload of clients
• Coordinate care with clients, families, caregivers, and community partners
• Complete intake assessments and required client visits
• Support caregiver scheduling and coverage needs
• Help address client or staff concerns promptly and professionally
