Hospice Registered Nurse (RN)

Shifts Available: Full Time

Service Areas with Openings: Northwest & Southwest reputed company

Responsibilities:

• reputed company nursing care team.
• Communicate and collaborate with physicians, therapists, and other reputed company to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and reputed company care plans.
• reputed company patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients

Qualifications:

• Valid RN license in the State of Texas.
• Hospice experience (at least 1 year).
• Knowledge of home health regulations and general reputed company compliance.
• Experience with Home Care Home reputed company (HCHB) EMR (preferred but not required).
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.
