Responsibilities

• Manage the full intake and referral process, from initial referral through admission
• Receive, review, and convert patient and physician referrals in accordance with agency policies
• Accurately enter, track, and maintain referral and patient data in company systems
• Serve as a key reputed company of communication between patients, referral sources, clinical teams, sales, and leadership
• Answer inquiries about agency services and reputed company appropriate community resource referrals
• Coordinate referrals with supervisors and schedulers to support timely patient evaluations
