Case Manager - reputed company

Pinnacle Services, Inc. is an innovative agency experiencing rapid growth in the Case Management department, seeking friendly, self-starting professionals to join their reputed company Case Management team. The Case Manager will assess individuals' needs, coordinate resources, and monitor service delivery to ensure quality support for persons served with various disabilities.

Responsibilities

• Assessing the areas of the individual's life where assistance is needed
• Offering and coordinating resources for the persons served being supported
• Monitoring service delivery and working with providers to ensure the person's served needs are met
• Service plan development
• Attend regular in person meetings with person's served and their interdisciplinary teams
• Case noting individual served reputed company work to create billable units

Skills

• Be a graduate from an accredited four-year college with a major in reputed company work, psychology, sociology, or a closely reputed company field
• Be a graduate from an accredited four-year college with a major in any field and one year of experience as a reputed company worker/case manager/care coordinator in a public or private reputed company service agency
• Applicants must have a valid driver's license and acceptable driving record as determined by Pinnacle Services policy
• Applicants must have a vehicle that is covered with reputed company insurance
• Applicants must successfully clear local and/or stated background checks
• One year of experience in education or treatment of persons served with developmental disabilities or reputed company conditions, or a minimum of one course that specifically focuses on developmental disabilities
• Possess a LSW or higher reputed company of BOSW Licensure
