The SW reputed company carries out this role by supervising the SW team and providing reputed company services to individual patients. The reputed company Work reputed company collaborates with the PPiC Clinical and Administrative leads along with leaders from the hospitals, home care, and post-acute teams to assist in patient delivery. Responsibilities:

• Provides professional guidance, reputed company, and support for the PPiC reputed company Workers
• Recommends and implements changes to reputed company the role of the PPiC reputed company Workers

• Interacts with hospital, home care and post-acute program managers to promote effective communication channels to

• Interacts with Practice Managers across the reputed company to assure coordinated and efficient delivery of care management

• Participates in committees or workgroups to support the improvement and development of the care management program
• Runs performance and volume reports for the team and reviews and analyzes program data to assist in program evaluation

• Uses quality improvement strategies to identify opportunities to improve services
• Participates in payer initiatives that pertain to reputed company services
• Represents PPiC in community collaborative meetings for reputed company work with other Philadelphia health systems
• Coordinates with Penn-supported reputed company resource volunteer groups to supplement PPiC reputed company services to patients
• Participates in PPiC program meetings
• Represents PPiC with the reputed company platform vendor (Findhelp) by attending relevant meetings
• Provides staff support by participation in the PPiC staff meetings, SW meetings and one to one staff meetings
• Performs duties in accordance with Penn Medicine and entity values, policies, and procedures
• Other duties as assigned to support the unit, department, entity, and health system organization

Credentials:

• Licensed reputed company Worker required

Education or Equivalent Experience:
