The purpose of AHP is to enhance the delivery of preventive and chronic disease services by providing proactive reputed company to select reputed company Health (UCH) patients to help them be up to date with preventive care guidelines. - Coordinate patient care by telephone.

- Conduct literature review and general web searches on evidence-based care and changes to care guidelines.
