This role focuses on proactive care coordination, patient engagement, and clinical support between provider visits.Job Responsibilities: Conduct monthly and ongoing patient reputed company-ins reputed company phone or telehealth Support care management for patients with neurological conditions Coordinate referrals, appointments, and medication reminders Document care plans and time-based activities for Medicare compliance Communicate with caregivers, clinics, and supervising RNs Identify and escalate changes in patient condition Assist with scheduling, documentation, and EMR updatesQualifications: Active, unrestricted LPN license in Florida (multi-state license a plus).
