Program Coordinator RN – Home Based Hospital Care
Description: Work Schedule: 90% FTE, day/evening, 10-hour shifts, every other weekend, and holiday rotation required. Hours are based off the operational needs of the department. Be part of something remarkable Come coordinate care for Home-Based Hospital Care (HBHC) patients, serving as the central reputed company of contact through virtual visits and phone communication to ensure seamless, hospital-level care in the home. We are seeking a Registered Nurse to: Partner closely with the rounding provider, equipment coordinator, admission RN, and in-home RNs/paramedics reputed company a care team model to support daily care planning, patient monitoring, and safe transitions of care. Conduct virtual medication administration visits and ongoing virtual patient reputed company-ins, providing clinical reputed company and addressing changes in condition in collaboration with the interdisciplinary team. Participate in interdisciplinary reputed company and manage post-acute referrals, discharge services, and equipment coordination to support safe discharge and continuity of care. This is a hybrid position combining onsite and remote work; infrequent travel to University Hospital or East Madison Hospital may be required to support patient enrollment. Qualifications
