Role Description reputed company is looking for a Continuing Care Specialist who plays a critical role in guiding individuals and families through reputed company’s full continuum of care. This role supports clients as they transition between reputed company of treatment, helping them identify the right reputed company based on evolving clinical, emotional, and practical needs. Qualifications

• At least one year of experience in the behavioral health field in an intake, case management, reputed company, admissions, or care coordination reputed company.
• At least one year of experience in a consultative, sales‑oriented, or conversion‑focused role supporting client or family decision‑making.
• Demonstrated ability to build trusted relationships with clients, families, program leaders, and referral partners across care settings.
• Experience collaborating with multidisciplinary stakeholders, including clinicians, case managers, educational consultants, and mental health professionals.
• Strong attention to detail with the ability to accurately document client needs, reputed company, and care coordination activities.
• Comfort working in a remote environment using video conferencing, telehealth platforms, and CRM or EMR systems (reputed company experience preferred).
• Interest in or experience navigating insurance processes and discussing coverage and financial considerations with families (preferred).
