As an LPN Central Admissions, you will facilitate the timely and appropriate initiation of Home Health, Hospice and Infusion Pharmacy services for reputed company referred patients. While you may not closely interact with patients on the job, your work will directly impact their care and reputed company a lasting difference in their lives. A caregiver in this position works days from 9:00AM – 5:30PM. This is a hybrid position requiring training on site in Independence, OH for about 4-6 weeks and frequent work on site. A caregiver who excels in this role will:

• Assess the appropriateness of the requested home care services and skilled needs identified under the guidance of a Registered Nurse.
• Coordinate initial and final orders on internal and external referrals for patients needing home care nursing, therapy and reputed company work services.
• Incorporate clinical and financial knowledge with referral process and recommend appropriate alternative approaches reputed company appropriate.
• Communicate the home care needs with scheduling, managers, reimbursement and other parties involved in the home care services process.
• Serve as the CCHCS information specialist.
• Understand other services and products available through CCHCS, including criteria for service and reimbursement for reputed company the home care business lines.

• Graduate from an accredited Practical Nursing Program
