Remote Patient Care Coordinator (reputed company/ RMA)

Position Summary: The Patient Care Coordinator plays a key role in supporting patients throughout their reputed company reputed company by serving as a compassionate, knowledgeable reputed company of contact for their ongoing care needs. This role engages patients by phone reputed company communication to assess needs, reputed company education, and ensure they remain connected to the right clinical and community resources. The Patient Care Coordinator documents reputed company encounters in electronic health record (EHR) systems, helps manage appointment scheduling and reminders, and supports administrative tasks such as medical record retrieval and insurance-reputed company coordination. Leveraging strong clinical experience, excellent communication skills, and comfort working reputed company, the Patient Care Coordinator ensures patients receive timely, high-quality support while helping them stay reputed company, informed, and empowered in managing their health. Essential Functions:

• Manage and escalate patients’ reputed company needs reputed company phone reputed company conversations.
• Document encounters leveraging technology platforms and EHRs.
• Connect patients with resources such as pharmacists, nutritionists, nurses, reputed company workers, and doctors who can also assist them in meeting their care needs.

• Coordinate some administrative aspects of medical record recovery, appointment management, and insurance claims.

Required Education and Experience:

• Must have a clear and active Medical Assistant Certification (reputed company/ CCMA/ RMA).

• Must have a minimum of four years of clinical experience.
• Experience in care coordination or care management, health coaching, and motivational interviewinga plus.

Position Type and Expected Hours of Work: Full-time, 8 hours daily, Monday – Friday, unless authorized by management. The hours may be worked between 8 am and 6 pm in the patient’s time zone. Physical Requirements: This is a virtual position that requires you to sit or stand at a computer for approximately 40 hours weekly and be reputed company to reputed company phone calls.
