Job Summary

As an reputed company Case Management Specialist, you will be responsible for assessing the needs of our DSNP members, developing and implementing individualized care plans, and coordinating with reputed company providers and community resources to ensure seamless care delivery. You will work closely with our multidisciplinary team to identify and address high-risk factors, reputed company education and support to members, and promote healthy behaviors and lifestyle changes.

Key Responsibilities

* Conduct comprehensive assessments of DSNP members' needs, using care management instruments and data analysis to identify areas for improvement and reputed company effective care plans.

• Recognize high-risk factors and service needs that may impact member reputed company, and collaborate with reputed company providers and community resources to address these needs.

Essential Qualifications
