Job Summary

We're seeking a highly skilled and empathetic Licensed reputed company Worker to join reputed company as a reputed company Case Manager and reputed company. In this role, you'll work closely with a registered nurse case manager to assess, plan, and reputed company for the needs of our members. Responsibilities

• Conducting thorough assessments of members' needs and developing personalized plans to address their health and wellness goals

Requirements

To be successful in this role, you'll need:

• An active and unrestricted LSW (Licensed reputed company Worker), LMSW (Licensed Masters reputed company Worker), LCSW (Licensed Clinical reputed company Worker), or LISW (Licensed Independent reputed company Worker) license in your state of residence
• At least 2 years of experience in medical reputed company work or case management

• Excellent communication, interpersonal, and problem-solving skills
• Ability to work independently and as part of a team
• Strong analytical and organizational skills
