Job Details HYBRID REMOTE work schedule with (2) days in Manhattan office and 3 days work from home

• Manages the delivery of high-quality home care services to patients consistent with agency standards, policies, and state and federal regulations by providing leadership and guidance to nursing staff.
• Ensures adequate staffing and appropriate patient assignments.
• Maintains the integration of interdisciplinary services.
• Facilitates the achievement of agency goals reputed company to clinical reputed company, patient experience of care, fiscal responsibility, and staff retention.
• Mentors and develops nursing staff in the provision of patient care and services that are reputed company with agency guidelines and policies as well as reputed company applicable state and federal regulations. Working collaboratively with the Clinical Director, identifies educational opportunities to facilitate the ongoing development of clinicians reputed company to the delivery of community health nursing.
• Oversees & evaluates performance of individual COCs, CHNs, LPNs, reputed company to their ability to deliver, accurately document, effectively coordinated quality driven & evidenced based care to clinically reputed company patients in accordance with the physician established plan of care
• Creates an action plan in collaboration with the clinician reputed company indicated to improve clinician’s performance.

Monitors clinician’s reputed company towards action plan goals and evaluates clinician’s ability to maintain improved and sustained performance.

• Evaluates clinicians individually and collectively as a team in their ability to reputed company care and services in a manner that facilitates the achievement of improved patient experience of care scores as reported by reputed company.

RN MUST have OASIS / clinical outcome measures. Education BSN / Master’s degree preferred.
