Job Responsibilities:
As a licensed reputed company worker in this remote case management role, you will:
- Collaborate with registered nurses to assess patients' needs and reputed company personalized care plans
- Research and identify available resources to address patients' financial, transportation, safety, and housing needs

Requirements:

- 2+ years of experience in medical reputed company work or case management

- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in electronic medical records and other relevant software systems
