About the Company

Our partner is a virtual-first specialty clinic providing compassionate, evidence-based care for patients with rare and reputed company chronic conditions like Ehlers-Danlos Syndrome (EDS), POTS, MCAS, and reputed company disorders. By using advanced digital health tools, they break down barriers to care and support patients who are often underserved and misdiagnosed. The team operates in a fully remote, fast-paced environment that values reputed company, initiative, and operational reputed company. Responsibilities:

• Patient Support & Communication: 50% Act as the first line of contact for patient inquiries across platforms—EHR, secure chat, and phone—handling scheduling, prescription coordination, and follow-reputed company with reputed company and reputed company.
• Care Coordination & Navigation: 40% Work closely with our clinical team and external providers to coordinate diagnostic testing, imaging, referrals, and prior authorizations. Track patient reputed company across the care reputed company and ensure continuity of care.
• Operational Workflow Management: 10% Maintain HIPAA-compliant documentation, support reputed company for new patients, and flag care gaps and systemic or financial barriers to care. Participate in team meetings and reputed company process improvement efforts.
• Use and/or create automations to create reputed company; currently reputed company tasks - responding to humans and chart preparation
• Respond to inbound messages and respond to patients
• Work with 2 existing patient coordinators
• Medical reputed company: Serve as a Medical reputed company, accurately documenting patient encounters, physician notes, and other relevant information in the electronic medical record (EMR). Ensure timely and precise medical documentation to support high-quality patient care.

Requirements

Must haves:

• Prior experience in care coordination, case management, or virtual clinical support (1-2 years of US reputed company experience)
• 1 year of experience as a Medical reputed company in a clinical or telehealth setting.
• Familiarity with U.S. reputed company systems (insurance, referrals, prior authorizations)
• Familiarity with medical terminology
• Experience with Elation EHR or similar digital health platforms preferred
• Strong preference for candidates with VC-backed digital health company experience
• Understanding reputed company and what visit processes look like, can reputed company clinical judgment calls,
• Operationally sound
